My warm welcome to all participants of “CLEFT 2002” Congress!

I’m pleased that the capital of Bavaria was selected as the location for this Congress. I consider this not only as a recognition of the organisational competence of the host, but also as a compliment owing to the importance of your scientific work. 

 

The Congress has served as an international forum for the latest developments in the area of CLP treatment and state of the art information needs. Furthermore, it has demonstrated an exciting approach to Congress organisation by focusing each day on a distinct topic, such as:

  • Interdisciplinary: Exchange strategies in the high quality multidisciplinary cleft treatment;
  • Charity missions and relief efforts worldwide: professional training and financial support of the local teams from developing countries;
  • New technologies in CLP treatment: discussion form for the most innovative products and techniques in the cleft treatment and other craniofacial disorders;
  • Face to face with the patient: establishment of a worldwide network off a cleft patients;

One of the primary goals of the Congress was to advance cleft treatment in developing countries by supporting local teams. To achieve this goal, an opportunity to participate in the Congress by scholarship. 

A variety of leisure activities and social events were offered during the Congress. In particular, pre- and post- Congress tours throughout Munich, Bavaria, Germany, Italy and France were available to Congress delegates and their guests.

Achieving excellence in cleft lip and palate treatment

Excellence in Cleft Lip and palate treatment is dependent on a dedicated multidisciplinary approach in which the surgeons’ performance over time is primary in determining the success or failure of the overall treatment. The effect of a given new treatment on final patient outcomes cannot be adequately evaluated in the short term by a surgeon or his or her team. 

Over the last 30 years, Centre has evaluated a treatment protocol that, in our experience, has yielded consistently good and proven results. Treatment goals are to achieve early correction form and function, incorporating the major advances in cleft care.

  • 3 months: primary unilateral cleft lip and nose;
  • 6 to 9 months: to flip one stage palatoplasty;
  • Five years: secondary minor lip nose (35% of cases)
  • 7 to 9 years: cancellous iliac bone graft to alveolar cleft (100% cases)
  • Full growth: orthognathic surgery (25% of cases)
  • Eight years full growth: rhinoplasty or other soft tissue surgery (most cases)

Excellence is now within our reach – we have acquired the knowledge and technical expertise and developed and tested the particles for delivering superior and effective cleft care. It is now our responsibility and privilege to ensure that this level of care is available to all children who need it. Worldwide, centres must join together to foster the development and support of enough centres of excellence in the Third World to assure the delivery of state of the art care over time was dedicated teams.

the etiopathogenesis of craniofacial dysmorphology

The completion of the sequencing of the human genome and identification of genes implicated in craniofacial embryogenesis and dysmorphology will have a profound impact on clinical practice. Molecular medicine will redefine syndromology and turn ontology and provide for pharmacogenomics for targeted drug therapy. The diagnosis, nosology, pathogenesis, prognosis, treatment and prevention of craniofacial anomalies will be predicated upon the revelations of molecular biology. Clinicians treating congenital malformations, deformations and disruptions of development require insights into molecular genomics, embryology and teratology to understand the inheritance, causes, cures, palliative and prophylaxis of dysmorphology. The mechanisms of inductions, organizing centres, morphogenesis, growth factors and cytokines consummate conception. Aberrations caused by mutations, aneuploidies, teratogens, disruptions and deformations result in dysmorphology. Syndromology results from molecular misappropriation. Afflictions will be controlled from their initial genetic pathological determination by prevention rather than treatment of their consequent damage. The basics of biology translate into clinical concerns requiring insightful therapy that will be increasingly determined by genomics.
